DOT PHYSICALS
What To Bring
For Drivers with Diabetes, Heart Disease, Sleep Apnea, or Medication Management of Anxiety, Depression or ADD/ADHD (Based on FMCSA Medical Examiner Handbook & federal guidelines).
Patients with Diabetes
If you take Insulin (Type 1 or Type 2):
- Completed Insulin-Treated Diabetes Mellitus Assessment Form — signed by your treating clinician within the last 45 days
- Recent A1C
- List of all diabetes medications and doses
Be prepared to show or discuss no severe hypoglycemia (loss of consciousness, seizure, or needing help from someone else) in the past 3 months.
If you do NOT take Insulin:
- Completed NON-Insulin-Treated Diabetes Mellitus Assessment Form — signed by your treating clinician within the last 45 days
- Recent A1C (within 3–6 months)
Patients with Heart Disease
(Heart Attack, Stents, Bypass Surgery, etc.)
- Letter (or provided form below) from your cardiologist stating:
- Your diagnosis, treatment, and current status
- That you are cleared to drive a commercial vehicle
- Recent cardiac test results, as recommended by your cardiologist:
- Stress test (within 2 years)
- Echocardiogram or EKG showing ejection fraction ≥ 40%
- List of all heart medications and confirmation you are tolerating them
- Hospital discharge summary if you’ve had a recent cardiac event
- Be aware of FMCSA required recovery periods before certification:
- Heart attack or post-MI stent: 2 months
- Elective stent: 1 week
- Bypass surgery (CABG): 3 months
Patients with Sleep Apnea (CPAP Users)
- CPAP compliance report from your machine or equipment provider showing:
- ≥ 4 hours per night on ≥ 70% of nights
- For at least 30 consecutive days
- Letter (or provided form below) from your sleep specialist (ONLY if newly diagnosed or severe) confirming:
- Your diagnosis and treatment plan
- That you are using CPAP as prescribed
- That you are safe to drive
- Be ready to confirm you have no daytime sleepiness or drowsy driving
Patients with Medicated Management of Anxiety, Depression, or ADD/ADHD
- Completed 391.41 CMV Driver Medication Form— signed by your treating clinician within the last 45 days confirming:
- Your diagnosis, treatment plan
This form is not always required before the exam & may also be requested by examiner after exam.
